Step #1 - Download The Dinner Reservation Form, Write in your Entree Choice for each attendee. Deadline is one week before the meeting. If you plan to attend but have not sent in a reservation and it is past the deadline, please email/call the Registration Director. Note that the email will only help get a dinner set up for you. The cost will be $40.00, and your entree will be "pot luck".
Step #2 - Use the online store to pay - www.ippgstore.org and make a note on the registration form that you have made your payment online. If you want to pay by check, enclose a check for the total amount made out to IPPG. Place the check and the form in an envelope.
Step #3 Mail the completed registration form the the address on the form. If you have any questions please contact the Registration Director.
The dinner reservation form requires Adobe Reader. If you can't see the form once it is downloaded or you get an error, download the FREE ADOBE READER first.
